Ted’s Tails Booking & Client Agreement Form

Ted’s Tails is a trading name of myself, Amy Blakey, operating as a professional dog care provider.

This agreement forms the entire contract between you (the client) and Ted’s Tails. By using our services, you agree to these Terms & Conditions along with the Client Registration Form. This agreement is governed by UK Law.Please read carefully. Your acceptance is confirmed by your signature or by engaging our services.

1. Booking Confirmation

  1. Bookings are only confirmed once payment has been received and the completed onboarding form is submitted. Provisional bookings cannot be held. Ted’s Tails is not responsible for holding dates if forms or deposits are not received within 10 business days of initial contact.

  2. All dogs must be fully up to date with vaccinations, flea/tick prevention, and worming. Proof must be provided prior to each booking.

  3. You agree to notify Ted’s Tails as soon as possible of any changes to dates or times. If the schedule changes during the stay, additional nights may not be available. If they are available, extra charges will apply and must be paid before the end of the booking.

  4. In the event of illness, personal emergency, or other circumstances preventing Ted’s Tails from safely completing the booking, we reserve the right to cancel the booking without liability.

  5. You acknowledge the inherent risks involved with transporting your dog (to/from your home, to the vet, or for walks) and agree to waive all claims against Ted’s Tails related to transport.

2. Your Dog(s) Details

  1. You must provide full, honest, and accurate details about your dog. 

  2. If your dog displays behaviour requiring safety measures (e.g., muzzles, gates, crates), you agree to these being used. Refusal may result in termination of service.

  3. If you consent to off-lead exercise, you accept full responsibility for third-party liability. Dogs will only be walked off-lead if it is safe and appropriate.

  4. You confirm you are the legal owner of your dog.

  5. You confirm your dog has not been ill with any contagious condition within the past 30 days and has received all required vaccinations.

  6. Ted’s Tails is not liable for consequential damages or damages beyond the replacement value of your dog.

  7. Dogs must wear a collar with an ID tag at all times, as required by law.

3. Veterinary Details

  1. Ted’s Tails prioritises your dog’s welfare at all times. If your dog becomes unwell or injured while in our care, we may take them to a vet. You consent to a qualified veterinary surgeon giving advice or treatment as necessary.

  2. You are responsible for all veterinary fees or pay the veterinary practice directly.

  3. While we will attempt to use your regular vet, this may not always be possible. Our default veterinary practice is Vets4Life Teddington.

  4. If your dog injures another dog or person, you accept full responsibility for all costs, including veterinary or medical bills. We recommend that your dog is insured for illness, injury, and third-party liability.

  5. All veterinary bills, regardless of cause, remain the responsibility of the owner.

4. Insurance

  1. Ted’s Tails is covered by Pet Business Insurance, including care, custody, and control of animals.

  2. This insurance does not cover veterinary costs or accidents caused by or to your dog.

  3. It is recommended, though not required, that clients have their own pet insurance.

5. Code of Practice

  1. Ted’s Tails operates with professionalism, honesty, and responsibility.

  2. Your dog’s welfare is our top priority.

  3. Dogs will not be left alone for more than 3–4 hours, unless otherwise agreed.

  4. While staying in your home, Ted’s Tails will maintain your dog’s normal routine, providing clean spaces, regular toilet breaks, fresh water, meals, and exercise.

  5. Dogs will be walked according to your instructions (on/off lead).

  6. Ted’s Tails will securely store all records needed for safe care, including contact details, feeding instructions, exercise needs, and veterinary information. 

6. Our Commitment to You

By completing and submitting this form, you provide Ted’s Tails with essential information regarding your dog’s health, behaviour, and care requirements.

Ted’s Tails commits to using this information to deliver services that prioritise the safety, wellbeing, and individual needs of your dog.

We pledge to treat every dog in our care with professionalism, attentiveness, and compassion, ensuring a reliable and personalised experience at all times.

7. Your Commitment to Us

As the client, you agree to:

  1. Pay the rates applicable at the time of service.

  2. Provide a minimum of 48 hours’ notice for cancellations to avoid charges.

  3. Supply all necessary information regarding your dog’s health, behaviour, and care requirements.

  4. Provide all necessary and safe equipment and supplies for your dog.

  5. Ensure access to your property, including providing keys or arranging entry.

  6. Authorise Ted’s Tails to seek emergency veterinary care if necessary. Every effort will be made to contact you beforehand. If unavailable, we reserve the right to use an alternative veterinary service.

  7. Cover all emergency or unforeseen expenses, including transportation, supplies, and veterinary fees.

  8. Accept responsibility for any injury or damage caused by your dog to any person or our staff and agree to indemnify Ted’s Tails accordingly.

  9. Ensure that if your dog is reactive, a properly fitted muzzle is worn at all times to protect your dog, other animals, and handlers. 

8. Your Pet’s Health and Safety

  1. All pets must be up to date with vaccinations, worming, and flea treatments.

  2. Dogs identified as reactive must wear a properly secured muzzle at all times while in our care to ensure the safety of the animal, staff, and public.

  3. Ted’s Tails reserves the right to terminate this agreement if your dog is deemed a danger to itself, other animals, people, or staff. In such cases, you authorise us to place your dog in alternative care at your expense.

9. Liability

  1. Ted’s Tails is not responsible for the security of your premises or for any loss if other individuals have access during the agreement period.

  2. If off-lead exercise is authorised, you accept full liability for any incidents resulting from your dog running away or causing harm.

  3. Ted’s Tails is not responsible for incidents arising from faulty or poorly fitting equipment.

  4. Ted’s Tails is not liable for damage caused by your pet during its time in our care.

  5. You acknowledge the inherent risks of daycare, home boarding, or group walks and agree not to hold Ted’s Tails or its staff liable for injuries.

  6. You agree to report any issues or concerns to Ted’s Tails within 24 hours of any appointment. 

10. Professional Development

Ted’s Tails is committed to continuous professional development to ensure the highest standard of care and safety for your dog. I have completed the Dog Walking & Sitting Academy (DWSA) Certificate Course, which includes:

  • Unit One: The Basics – Heelwork, introductions, and equipment

  • Unit Two: Canine Communication & Body Language

  • Unit Three: Pack Structure & Dominance (modern understanding and practices)

  • Unit Four: Dog Law

  • Unit Five: Business Management

  • Unit Six: Canine First Aid

  5. If you return earlier than the agreed end date, the full agreed rate still applies.

  6. You agree to provide enough food and supplies for the duration of the stay. If additional food or items must be purchased, you agree to reimburse the cost.

  7. If you do not return to your home or make contact within 14 days of the agreed end date, despite all reasonable attempts to reach you and your emergency contact, we will contact the appropriate authorities or welfare organisations.

  8. You acknowledge the inherent risks involved with transporting your dog (to/from your home, to the vet, or for walks) and agree to waive all claims against Ted’s Tails in relation to transport.

2. Your Dog(s) Details

  1. You must provide full, honest and accurate details about your dog in all forms. Any undisclosed behavioural or medical issues that affect suitability for in-home boarding may result in immediate termination of the stay.

  2. If your dog displays behaviour requiring safety measures (e.g., muzzles, gates, crates), you agree to these being used. Refusal may result in termination of service.

  3. If you consent to off-lead exercise, you accept all responsibility for third-party liability. Dogs will only be walked off-lead if safe and appropriate.

  4. You confirm you are the legal owner of your dog.
